Multnomah County Health Department                                                      
Program: HIV Prevention Program

Description of Program: The mission of the Program is to prevent the spread of HIV infection. The activities of the HIV prevention program consist of HIV counseling and testing, distribution of prevention items (bleach, needles and condoms), needle exchange, group education, contracts with community agencies, and staffing the Multnomah County HIV Prevention Community Planning Team. HIV testing is available at the Multnomah County HIV Community Test Site, Multnomah County NE Health Center, and at all Multnomah County Health Department Clinics.

Northwest Portland Area Indian Health Board                                         
Program: Project Red Talon

Description of Program: The Project provides STD testing, screening and treatment services among Northwest tribal clinics. They work to strengthen the capacity of tribal health educators, program managers, and clinicians to provide STD prevention services to the Northwest tribes. The Red Talon project works to increase community awareness about STDs and HIV. Project Red Talon serves the 43 federally recognized tribes in Idaho, Oregon and Washington.

Oregon Health and Science University                                                      
Program: OHSU Partnership Project

Description of Program: Oregon Health and Science University (OHSU) Partnership Project renders an array of HIV/AIDS services including case management and nursing case management to individuals affected by HIV/AIDS in Multnomah, Washington, Clackamas, and Columbia Counties, community education, chronic disease self-management, and services to those at-risk.  Partnership Project’s prevention arm is Supporting Healthy Options for Prevention (SHOP) which offers individualized client-centered risk reduction counseling based on the CDC’s CRCS model.  The program service area for SHOP is statewide, offering both 1 on 1 and phone based counseling services free of charge.  Partnership Project’s mission is to assist those infected by HIV/AIDS to obtain high quality health care, treatment, information and support.
